Election Forms

Description
Registered voters may vote by absentee ballot if they will be absent from the Town during the entire peiod the polls are open on Election Day or if a physical disability prevents such voter from being present and personally voting at the polls. You must file for an Absentee Ballot not later than seven (7) days before the election. Fill out the Application and bring or mail it to the Town Office at 6401 Forest Road, Cheverly, MD 20785
Within seven (7) days of a Town Election and on the day of the Election prior to the time the polls close a qualified voter may vote by emergency absentee ballot, if such voter was informed within that 7 day period that they must be absent from the Town while the polls are open. - OR - If a qualified voter is unable to be physically present at the polls as a result of illness or accident occurring within wuch 7 day period - OR - because of a death or serious illness in such voter's immediate family occurring after the beginning of such period.
All Candidates for office must submit this statement with their filing petition. All elected officials and department heads submit this statement every year.
This form must be signed and brought in to the Town Clerk. All candidates for an Elective Office must submit this form with a disclosure statement and bring documentation (Passport / Drivers License / Social Security Card / Birth Certificate, etc) and fill out US Dept of Justice Form I-9, to establish both Identity and Eligibility.

Description
A Residential Business License is required for operation of a business in your home. It is renewable annually. There is a $20 annual fee.

Call the Town Office about acceptable businesses in R-55 residential zoning.

The form must be filled out, signed, and returned to the Town Office.
This is a maintenance exception to the Alleyway Policy. The form must be filled out by all affected property owners and returned to the Town Office.
A Reservation Use Agreement Permit is REQUIRED for any group with over 30 persons using the Pavilions in Town Park or Boyd Park.

Pavilions may only be reserved by a CHEVERLY resident for a gathering of family or friends or a Cheverly club or group with at least 50% resident membership.
The Cheverly resident responsible for reserving the Pavilion MUST be present at the Event, or reservation will be revoked and the group will be asked to leave, by the Police.

Park Reservation Fee is $30 (non-refundable). Cheverly resident reserving park MUST show valid Maryland Driver's License (or other valid ID with Cheverly address) in person at Town Office before reservation will be made.
A resident may use this form to gather signatures to petition the Town Council for the installation of various traffic calming devices.

This permit is needed for any stationary, temporary sales location. It requires a $2,000 bond and a $25 per day fee.
